For your convenience, our office accepts many forms of payment including Visa, cash and check. We do not accept medicaid. To find out whether your insurance plan will cover your procedure or consultation it may be necessary for you to contact them. We process your insurance forms for you as a courtesy, however, payment in full is ultimately the responsibility of the patient or guardian. Our office has eliminated billing, therefore, payment in full is requested at the time of the service rendered.
